PERSON-CENTRED SUPPORT

Support shaped around
what matters to you.

Small practical tasks can make a significant difference to comfort, confidence and independence. We agree which activities require support and which the person wishes to continue doing themselves.

Before care begins, we take time to understand needs, routines, preferences, risks and desired outcomes. Support is then recorded in an agreed care plan and reviewed when circumstances change.

WHAT SUPPORT CAN INCLUDE

Practical help,
delivered with dignity.

Every care arrangement is individual. Following assessment, support may include:

Meal and drink preparation
Shopping and putting groceries away
Light household routines connected with care
Support with letters, calendars and appointments
Accompaniment to essential appointments
Encouragement with hydration and established routines
WHO IT MAY HELP

Is this support suitable?

This support may help someone recovering from illness, living with reduced mobility or finding everyday activities increasingly difficult without regular assistance.

HOW WE WORK

Safe and clearly planned

Tasks are clearly recorded in the care plan and completed within the agreed visit time. We monitor changes in wellbeing and raise concerns through the appropriate route.

COMMON QUESTIONS

Things you may
want to ask.

We will discuss your circumstances directly and explain what can safely be provided.

Does this include general cleaning?

Our focus is domiciliary care. Light household tasks connected with the person’s daily care may be agreed, but this is not a standalone cleaning service.

Can carers prepare meals?

Yes. Meal and drink preparation can be agreed according to preferences, dietary information, risks and the care plan.

Can you take someone to an appointment?

Accompaniment may be possible following assessment, travel planning and confirmation of the time required.
