PERSON-CENTRED SUPPORT

Support shaped around
what matters to you.

Care may be needed at one particular point in the day or across several regular visits. We discuss the appropriate visit pattern, what needs to be achieved and the time required to provide support safely.

Before care begins, we take time to understand needs, routines, preferences, risks and desired outcomes. Support is then recorded in an agreed care plan and reviewed when circumstances change.

WHAT SUPPORT CAN INCLUDE

Practical help,
delivered with dignity.

Every care arrangement is individual. Following assessment, support may include:

Morning and bedtime routines
Regular daytime check-ins
Meal-time support
Planned welfare and wellbeing visits
Short-term support following a change in circumstances
Reviews when needs or preferred routines change
WHO IT MAY HELP

Is this support suitable?

Flexible visits may suit people who need regular help at particular times while continuing to manage other parts of daily life independently or with family support.

HOW WE WORK

Safe and clearly planned

Schedules are agreed transparently and depend on assessment, travel, staffing and availability. Contingency and escalation procedures guide our response when arrangements are disrupted.

COMMON QUESTIONS

Things you may
want to ask.

We will discuss your circumstances directly and explain what can safely be provided.

How long is each visit?

Visit length is agreed after assessment and must allow enough time for the planned support to be delivered safely and without rushing.

Can the schedule change later?

Yes. We review care when needs or circumstances change and agree any safe adjustment to visit times, frequency or duration.

Can you guarantee an exact arrival time?

We agree a planned schedule and communicate significant disruption, but travel, emergencies and the needs of people receiving care can sometimes affect arrival times.
