Returning home can involve new routines, reduced confidence or temporary support needs. We assess what is required at home and agree how our team can complement the discharge plan.

Before care begins, we take time to understand needs, routines, preferences, risks and desired outcomes. Support is then recorded in an agreed care plan and reviewed when circumstances change.

Safe discharge requires coordination. With appropriate consent, we work from available discharge information and communicate with authorised relatives and involved professionals when necessary.

How quickly can support start?

This depends on assessment, available information, the person’s needs and suitable staff capacity. Early contact before the planned discharge date is helpful.

Can support be temporary?

Yes. Care can be reviewed as recovery progresses, with visits adjusted or ended through an agreed and safe process.

Do you replace NHS clinical services?

No. We provide agreed domiciliary care and work alongside, rather than replace, services that require a registered healthcare professional.
